Physicals are mandatory in order to pick up your players equipment.  As you get them, please make sure to upload the completed forms to your account. (See directions below)

Please make sure you download and fill out the parent/player info portion prior to your visit to the doctor and then upload a completed copy of your physical form to your account.  If the player has had a physical in the past six months, most doctors will complete this form without a new appointment, so make sure to ask their office.  If you would like to pay cash without an appointment, try Medallus Medical (476 N 900 W American Fork, UT 84005). They are open 7 days a week from 9-8pm and charge $25 cash.  Please note, if your child has preexisting conditions, such as cardiac history or a birth defect,etc. please take documentation to your physical at this location so they can be cleared to participate.

 To upload a physical form to an account:

1. go to westlakeyouthfootball.com

2. login to your account

3. click on "my account" in the upper left corner

4. click on "members"

5. select the pencil icon next to the player's name

6. scroll down to Physical Form and click "select"

7. upload the photo of your player's physical (make sure it is the page with the doctor's clearance)

8. click "save" - this MUST be done or the form will not save in your player's account.
